New Patient Health Form

Welcome to Highland Family Dentistry!  We want your first visit to be a pleasant and smooth one.  If you have dental insurance, please have your card handy so we can verify coverage ahead of time and assist with any claims, as well as inform you of your plan's benefits and limitations.

You will need to complete a Medical History Form before treatment.  One is provided that you can download and fill out ahead of time.  If you would like to include dental records from your previous dentist into your new dental record, you can bring them with you at your first visit, or you can have us request them from your doctor.

At your first visit, we will need various x-rays to be able to adequately provide a complete and thorough examination.  We will take only those films requested to accomplish this.  Unless your first visit is an emergency, we will then begin your cleaning.  If you have been having regular preventive dental treatment, your cleaning will probably take only one visit.  If not, more than likely you have gum inflammation with tartar buildup.  In this case, you might need what is called debridement and your cleaning will require subsequent visits.  The doctor will give you a complete examination and diagnosis.  Your oral condition will be discussed, and any questions about your case will be answered then.  Arian or Catherine will arrange an appointment to begin treatment and go over all financial arrangements with you in advance.
